Pre-Exercise Medical Questionnaire

Before you enter the Gym, you must read through and answer the following:

• Has a Doctor/Medical Professional ever diagnosed you with a heart condition and indicated you should restrict your physical activity?

• When performing physical activity, do you feel pain in your chest?

• When you were not engaging in physical activity, have you experienced chest pain in the past month?

• Do you ever faint or get dizzy and lose your balance?

• Do you have an injury or orthopaedic condition (such as a back, hip, or knee problem) that may worsen due to a change in your physical activity?

• Do you have high blood pressure or a heart condition in which a Doctor / Medical Professional is currently prescribing a medication?

• Are you pregnant?

• Do you have insulin dependent diabetes?

• Are you 69 years of age or older and not used to being very active?

• Do you know of any other reason you should not exercise or increase your physical activity?

If you have answered “yes” to any of the above questions you will not be able to enter the Gym. If you are in any doubt, you should obtain medical advice before starting any exercise programme.


Declaration

I confirm that I understand that there are health risks to undertaking exercise, that you have recommended that I should consult my doctor before undertaking a programme of exercise and if not experienced in using gym facilities, that I have an induction on the use of such facilities.

I have completed the Pre-Exercise Medical Questionnaire accurately and honestly.

I understand and accept that you operate CCTV cameras at the Gym and that the recording and storage of images may occur in accordance with your policy.

I accept that you do not accept responsibility for any accident, injury, illness or misadventure caused to or suffered by visitors to the Gym unless caused by your wilful default or negligence, or that of your agents.

I have read the terms of entry on the reverse and agree to abide by them. Whenever I return to the Gym, I agree to all of them and you may refuse entry to me where you consider it appropriate for the proper running of the gym.


TERMS OF ENTRY TO NOS DA GYM

The Gym is not supervised other than for compliance with these terms of entry and you may enter only on the basis of you accept that you do so at your own risk.

To ensure maximum benefit from your use of the Gym and to increase your safety, we strongly recommend that you undertake a supervised induction whatever your physical condition. Please ask for further information on when and where this may be available.


Entry fees

Please see our separate notice available at reception setting out our current rates of entry fee. Entry will only be permitted to the gym on payment of the appropriate entry fee and upon your signing the declaration on the front of this sheet. Monthly membership begins on the day payment is received and runs until the same day the following month. Payment is non-refundable. The membership card must be shown on each visit, or the usual single entry fee will apply. If the card is lost or stolen, no replacements will be issued, and the usual single entry fee will apply.


Lockers

We provide lockers, but we do not guarantee the availability of a locker to you at the time of your visit. We do not accept responsibility for any damage to or loss of your property except to the extent it is caused by any negligence on our part.

The lockers will be checked and emptied whenever the Gym is not in use and any contents stored for 7 days before being disposed of as abandoned goods.


Equipment

We will use our reasonable efforts to ensure that the equipment in the Gym is maintained in good working order. We do NOT guarantee that:

• all equipment in the Gym will be in working order at all times.

• any particular equipment will be available for your use.

We may carry out repairs, maintenance or alternations without prior notice.


Conduct

Any visitor making use of our facilities is responsible for ensuring that he or she is properly dressed and equipped and that he or she is in sufficiently good health such as to avoid any undue risk to himself or herself or others visiting the Gym.

You must use the equipment in the manner for which it is intended and in accordance with any notices suitably displayed. You must not abuse the facilities or equipment and will pay for any damage that you wilfully or negligently cause.

You will not behave in any way that is inappropriate whether in relation to the use of the equipment or in respect of other users of the Gym or hotel staff. You accept that you will leave the Gym forthwith without refund of entry fee if so asked.

You must not enter the Gym if you are suffering from any infectious or contagious illness, disease or other ailment where there may be any risk (however slight) that your use of the gym may be detrimental to the health, safety, comfort or physical condition of any other user of the Gym.
